It is the world’s largest health assurance scheme, launched in September 2018, and covers more than 12 crore poor and vulnerable families.

Eligibility Criteria

- Aadhaar card (or any government ID)
- PM-JAY card / Golden Card (issued after enrolment)
- SECC verification or ration card details
How to Apply
- Visit pmjay.gov.in and check eligibility with your mobile number
- Visit a nearby Common Service Centre (CSC) or empanelled hospital for enrolment
- Get your PM-JAY Golden Card printed at any CSC for a small fee
- At the hospital, show your card and the hospital handles the rest — cashless treatment
